ADA ACCOMMODATIONS


The Americans with Disabilities Act (ADA) provides that an employer must make reasonable accommodations* which allow otherwise qualified employees with disabilities to perform essential duties of their position unless it causes undue hardship on the employer.

Option 1: Fill out the online request form found below. Your request will be routed to the Absence Management team in University Human Resources and representative will be in contact with you.

You may be required to provide the Health Care Provider Statement, provider documentation of accommodation requirement or work  arrangement necessary for completion of this form by either:

Fax to 706-542-7321 OR

Deliver in person to Office of Human Resources – attn. to: ADA . 
Please use HR Drop box on the front of the building: 215 S. Jackson Street Athens GA 30602   OR

USPS Mail – attn. to: ADA at 215 S. Jackson Street Athens GA 30602

Once one of the options above has been completed and the information has been received, your request will be routed to the Absence Management team in University Human Resources, and a representative will be in contact with you.

 

 


REASONABLE ACCOMODATION EXAMPLES 

 

Making facilities readily accessible and usable

Reallocating or redistributing marginal job functions

Altering when or how essential functions are performed

Use part-time or modified work schedules

Obtaining or modifying equipment or devices

Reassign to a vacant position

Permitting use of accrued paid or unpaid leave

Providing reserved parking

Allowing employee to provide devices

 

 


 PROHIBITED PRE-EMPLOYMENT INQUIRIES 

 

Have you ever been treated for listed diseases?

List any conditions or diseases for which you have been treated in the past.

Have you ever been hospitalized?

Is there any health-related reason you may not be able to perform the job for which you are applying?

How many days were you absent from work because of illness last year?

Have you ever been treated for drug addiction or alcoholism?

Have you ever filed for workers’ comp?

 

 


PERMISSIBLE PRE-EMPLOYMENT INQUIRES 

 

Qualifications of applicant to perform the job

Describing the requirements

Asking applicant if he/she can perform the duties of the position

Asking applicant to describe or demonstrate how he/she will perform the functions

Describing company policies such as attendance, timeliness reporting to work, work hours, etc.

Asking applicant if he/she can meet these requirements

If applicant self-identifies as a person with a disability, it is permissible to ask if he/she can perform the essential job duties with or without an accommodation
