Benefits open enrollment details for active employees and retirees with USG health, dental vision and/or life insurance plans.

We will have an active enrollment this year.  All employees/retirees and dependent age 18 or over covered on USG health plans must certify/re-certify their tobacco status.  You must enroll each year if you want a flexible spending account (FSA). Health Savings Accounts (HSA) will roll over.

Tobacco surcharge - ACTION REQUIRED
Per the University System of Georgia (USG), the monthly tobacco surcharge will remain at $75 and will apply to all health insurance covered employees, retirees, and health-covered dependents (spouse and/or children) age 18 or over who use tobacco products.  All employees/retirees and dependents 18 or older covered on USG health plans must certify/re-certify their tobacco status.  Anyone who fails to re-certify will default to tobacco user and the $75 tobacco surcharge will be added to the premiums for the employee/retiree and eligible dependents.  Although no surcharge refunds will be permitted, non-tobacco users can have the surcharge removed during 2019 by using the OneUSG Connect Benefits system to indicate they or their family members no longer use tobacco products.

During open enrollment (October 29 - November 9), you may: 

Health -Enroll, change plan, add or drop eligible dependents, or cancel coverage.
-May add eligible children below the age of 26, regardless of student or marital status.
-If you are adding eligible dependents, you must provide required documentation.  This includes documents such as a government issued birth certificate, a marriage license and a joint account etc.  Enroll, change, or cancel coverage using OneUSG Connect - Benefits.
Dental -Enroll, add, or drop eligible dependents, or cancel coverage.
-Switch between the "High" plan and the "Base" plan or vice versa. 
-If you are adding eligible dependents, you must provide required documentation. This includes documents such as a government issued birth certificate, a marriage license and a joint account etc. Enroll, change, or cancel coverage using OneUSG Connect - Benefits.
Vision insurance Enroll, change, or cancel coverage. If you are adding eligible dependents, you must provide required documentation. This includes documents such as a government issued birth certificate, a marriage license and a joint account etc. Enroll, change, or cancel coverage using OneUSG Connect - Benefits.
Shared Leave Program If you are already a member of the shared leave pool, you are NOT required to donate sick leave during open enrollment to remain in the pool.  However, you may donate additional hours if you wish.   If you are not a member of the shared leave pool and wish to join, you must donate a minimum of 8 hours sick leave (prorated for part-time employees) during open enrollment. Donation instructions will be provided on October 26.

Short Term and Long Term Disability Apply for coverage in 2019 using OneUSG Connect - Benefits. May cancel coverage at any time.
Medical flexible spending account
Enroll for 2019 (must enroll each year to participate) using OneUSG Connect - Benefits.
Dependent care flexible spending account

Limited purpose flexible spending account  ONLY for participants with the consumer choice health plan and health savings accounts. Must enroll each year to participate. To be used for eligible vision and dental care expenses only. Enroll using OneUSG Connect - Benefits.

Adding dependents to your coverage

You must provide supporting documentation in order to add dependents to insurance coverage. If you do not provide this documentation, your dependent may not be added to your coverage. This includes documents such as a government issued birth certificate, a marriage license and a joint account etc.

Some 2019 health plan details:

In 2019, the University System of Georgia will continue to offer four health care plan options to our active employees and our pre-65 retirees and pre-65 retiree dependents. For each Medicare eligible retiree and/or dependent enrolled in coverage through the Aon Retiree Health Exchange, USG will contribute $2,736 annually to a Health Reimbursement Account (HRA).

Changes to our health care plans:

  • Anthem Blue Cross & Blue Shield Comprehensive Care Plan:
    • Increase in-network deductible from $500 to $750 (EE) and $1,500 to $2,250 (FAM)
    • Increase out-of-network deductible from $1,500 to $2,250 (EE) and $4,500 to $6,750 (FAM)
    • Increase in-network OOPM from $1,500 to $1,750 (EE) and $3,000 to $3,500 (FAM)
    • Increase out-of-network OOPM from $3,750 to $5,250 (EE) and $7,500 to $10,500 (FAM)
  • Consumer Choice HSA:
    • Increase in-network deductible from $2,000 to $2,200 (EE) and $4,000 to $4,400 (FAM)
    • Increase out-of-network deductible from $4,000 to $4,400 (EE) and $8,000 to $8,800 (FAM)
    • Increase in-network OOPM from $3,500 to $3,700 (EE) and $7,000 to $7,400 (FAM)
    • Increase out-of-network OOPM from $7,000 to $7,400 (EE) and $14,000 to $14,800 (FAM)
  • Blue Choice HMO:
    • Physician Office copay $30 to $35 - Specialist copay $60 to $70
    • Outpatient Hospital $200 to $250 copay
    • Urgent Care $60 to $70 copay
  • Comprehensive Care & BlueChoice HMO:  Increase Retail Rx copays:
    • Generic:  $10 copay to $15 copay
    • Preferred Brand:  $35 copay to $40 copay
    • Non-Preferred Brand:  20% with $45 min and $125 Max to 20% with $50 min and $130 Max
  • Tobacco certification:  All employees/retirees and eligible depends age 18 or older are required to certify/re-certify their tobacco status
  • Health Savings Account (HSA):  Individual annual contribution limit will increase from $3,450 to $3,550 and the family annual contribution limit will increase from $6,900 to $7,000

Consumer Choice HSA health plan and HSA employer match money

  • The Consumer Choice HSA plan has low premiums, high deductibles, and allows the use of health savings accounts (HSA) — pre-tax savings accounts for eligible medical expenses. HSAs roll over from year to year.
  • The University System of Georgia has again contracted with Optum to provide HSAs for all System institutions. View more information about the Optum HSA
  • Participants who are in or enroll in the Consumer Choice HSA plan during open enrollment and contribute to an Optum health savings account (HSA) will receive employer match money in 2019. The employee’s HSA contributions will be matched, dollar for dollar, up to $375 for individual coverage and $750 for more-than-individual coverage. In order for employees to be eligible to receive the match, the employee must make contributions to an Optum HSA via payroll deduction. Attention UGA departments: funding sources (grants, etc.) for the match may vary.
  • For 2019, the University System of Georgia will again pay the Optum monthly administrative fees.
  • HSA contribution limits for 2019: individual $3,500; family $7,000; catch-up contribution (age 55 or over) $1,000.

This service is available to Comprehensive Care, Consumer Choice HSA, and BlueChoice HMO participants. CVS Minute Clinics

This service is available to Comprehensive Care, Consumer Choice HSA and BlueChoice HMO participants. Clinics are located in select CVS/pharmacy® stores nationwide for service at a reduced price ($15 co-pay for Comprehensive Care and BlueChoice HMO participants; reduced negotiated rate for HSA participants). ID cards

BCBS is changing their name to Anthem Blue Cross Blue Shield.  New ID cards will be mailed to employees, and retirees enrolled in Anthem Healthcare plans in 2019.  As a reminder, employees and non-Medicare eligible retirees enrolled in a BCBSGa plan will not receive a separate card for their pharmacy benefit. Medical and pharmacy information is included on one card.

Well-being Resources

Well-being programming is a major component in improving the health and well-being of our employees and campus communities. The USG system office in coordination with the Total Rewards Steering Committee (TRSC) well-being subcommittee will continue to provide direction, resources and support for well-being programming on the campuses. In 2019, employees and spouses will continue to have the opportunity to earn a well-being incentive of up to $100 each for participating in well-being activities. 

Shared Leave Program

  • If you are already a member of the shared leave pool, you are NOT required to donate sick leave during open enrollment to remain in the pool. However, you may donate additional hours if you wish. If you are not a member of the shared leave pool and wish to join, you must donate a minimum of 8 hours sick leave (prorated for part-time employees) during open enrollment. You must have at least a 48-hour sick leave balance on January 1, 2019 in order to enroll in the Shared Leave Program. You must have a balance of 40 hours of sick leave after donated leave is taken from your balance in January 2019. Your donation can only be made during open enrollment and does not preclude you from receiving more than 8 hours of shared leave in 2019, if you are approved.
  • Instructions on how to donate sick leave will be provided on October 26.

Dental Insurance

The University System of Georgia (USG) has again contracted with Delta Dental for 2019. Dental premiums will remain the same in 2019. The annual benefit maximum for the High plan (see choices below) will remain $1,500 for 2019. The benefit maximum for the Base plan will be $1,000 per year. You may cover eligible family members; however, if you are adding new dependents, you will be required to provide dependent documentation. This includes documents such as a government issued birth certificate, a marriage license and a joint account etc.

TWO DENTAL PLAN CHOICES:
You may choose the High plan or the Base dental plan. The Base plan has lower premiums and coverage. View details about the 2019 dental plan. Find dental providers.

Be sure to select the "Delta Dental PPO" network for the greatest savings.

Life Insurance

The University System of Georgia (USG) has again contracted with Minnesota Life for 2019 basic and supplemental life insurance. You are encouraged to enter your beneficiary designations in OneUSG Connect - Benefits. Employees may increase optional life insurance coverage by 1x your salary up to a limit of 3x your salary or $500,000 (whichever is less) without a statement of health (EOI). If you increase your coverage to above this amount, a statement of health is required. You may purchase up to 8x your salary with a statement of health. View details about the life insurance plan.

You may cover eligible family members; however, if you are adding new dependents, you may be required to provide dependent documentation (birth certificate, marriage license, etc.). Spouses may NOT be covered on spouse life insurance if they are eligible for employee coverage. You may cancel supplemental life insurance at any time by contacting OneUSG Connect-Benefits at 1-844-587-4236.

Any increase in spouse life insurance will require a statement of health. Coverage will show as "pending" until the statement of health is received and reviewed by Minnesota Life.

The University System of Georgia has again contracted with Optum to administer the flexible spending account program (FSA) for health and dependent care reimbursements. Using Optum, you'll enjoy features, including:

  • 24/7 online access to your account information
  • Debit spending card for medical expenses (card fee paid by UGA)
  • An additional 2 1/2 month grace period to incur expenses and submit claims
  • Toll-free customer service phone center Monday through Friday, excluding holidays, from 8:00am to 8:00pm (ET) at 877-470-1762.

The dependent care spending account allows employees to pay for eligible dependent care expenses with before-tax dollars by allowing pre-tax contributions to a flexible spending account. Contribution limit for 2019: $5,000 or $2,500 if married filing separately.

The health flexible spending account allows employees to pay for eligible health care expenses such as co-pays, deductibles, co-insurance, and medications. Contribution limit for 2019: $2,600.

The limited purpose flexible spending account (LPFSA) is available only to contributors to 2017 Optum health savings accounts. The LPFSA is for dental and vision care expenses only and coordinates with the HSA.  Contribution limit for 2019: $2,600.


Vision Insurance

EyeMed will continue to provide vision insurance for USG institutions  If you choose to buy vision insurance, it will supplement any vision benefits included in your health plan, if applicable. Review the EyeMed vision insurance. Find an EyeMed provider.

(select the "Insight" network and type in your zip code). You may cover eligible family members; however, if you are adding new dependents, you will be required to provide dependent documentation. This includes documents such as a government issued birth certificate, a marriage license and a joint account etc.

Short-Term Disability Insurance

MetLife will again provide STD insurance for 2019. Short-term disability insurance provides 60% salary replacement if you cannot work due to illness or injury for up to 11 weeks.

The cost of the MetLife STD insurance is based on 60% of your weekly benefit amount. The rate for STD will not change for 2019: $.291/$10 of covered benefit. For example, if your gross weekly salary is $600, your monthly premium would be (60 x .60) x .291 = $10.48.

You will begin receiving income benefits once you are out of work for more than 14 calendar days.

Long-Term Disability Insurance (LTD)

MetLife will again provide LTD insurance for 2019. Long-term disability insurance provides 60% salary replacement in the event you are unable to work for at least 90 days due to serious illness or injury. The cost of the plan is based on your salary. The rate will remain the same for 2019- .266/$100 of covered salary.

For example, if your gross monthly salary is $2500, your monthly premium would be $25 x .266 or approximately $6.65 per month. Per LTD plan rules, individuals with pre-existing conditions are not allowed to file claims for that condition for 365 days after the effective date of enrollment.

Accidental Death & Dismemberment Insurance (AD&D)

The University System of Georgia has again contracted with Securian Life (formerly Minnesota Life) to provide AD&D coverage (in addition to the AD&D insurance included with your life insurance). During open enrollment, you may enroll in the AD&D plan with no statement of health required. You may also change your coverage amount, change your level of coverage (family vs. individual), change your beneficiary, or cancel your coverage.
